Why am I so tired in perimenopause? Start with the first two minutes of your day

Quick answer

For many women, perimenopause shows up as exhaustion, irritability and brain fog long before hot flashes. Before you train harder or eat less, start with the first two minutes after you wake up: phone down, five slow breaths, a glass of water, then morning light.

The old story was hot flashes. My doctor told me I needed them before I could go on hormones.

The reality is, that’s not what I felt. I felt fatigue dragging me through the day. New irritability before my cycle. Lighter sleep, and waking up at 2 or 3am. And brain fog that went way past mommy brain: forgetting names, forgetting why I walked into the room, hunting for sunglasses that were on my head.

I wasn’t the exception. In a large 2026 survey of more than 12,000 women, exhaustion, irritability and brain fog ranked above hot flashes.

The first two minutes

When the energy dips and the fog rolls in, most of us push. Longer workouts. Smaller plates. A voice in our head meaner than anyone we know. That’s more stress on a body already running on less.

So start smaller. Start in bed. Those first minutes set the tone for your stress hormones and your body clock. Research points to slow breathing and morning light as two of the simplest ways to steady both.

  • Phone stays down. No inbox, no news, no scroll until your feet are on the floor.

  • Five slow breaths, one kind sentence. In through the nose, longer out.

  • Water, then light. A full glass before coffee. Then open the blinds or step outside.

Your 7-morning test: by morning seven, notice your 2pm. Clearer head? More in the tank?

When you start hormones may matter

For twenty years, most of us heard one thing about hormone therapy: risky. That fear came from a 2002 study in which many women started hormones about ten years after menopause.

In November 2025 the FDA removed the black-box warning from hormone therapy, and new labels talk about timing: starting before 60, or within ten years of your last period. A September 2026 study in JAMA Internal Medicine followed more than 2,700 women for twenty years and found fewer heart attacks, strokes and heart failure in women who started early, and no benefit in women who started later.

These studies show a pattern, not proof. Not every woman is a candidate, and hormones don’t do the daily work for you. That’s why hormones are Step Four in The Well Drop Method. Habits, home and data come first.

Women do great things in the wrong order. Last in the order. Not late in life.

Questions women ask

What are the first signs of perimenopause? For many women, the first signs are fatigue, irritability, lighter sleep, waking at 2 or 3am and brain fog, often years before hot flashes.

Is it normal to wake up at 3am in perimenopause? Many women notice lighter sleep and middle-of-the-night waking as hormones shift. A steady morning routine and a wind-down at night are simple places to start. Talk to your clinician if it persists.

Should I start with hormones or habits? In The Well Drop Method, habits come first, then your home, then your data, and hormones last. A hormone works best on a body that’s already supported. Talk to your clinician about whether hormone therapy is right for you.
